I'll admit, I'm not exactly a paragon of great fitness - however, I do like to do this hike once in a while.
It is a challenge, with over 1000 railroad ties--ladder steps straight up.
The halfway point is an old railroad trestle, which can be tricky to negotiate for a number of reasons; one is that there is a bee's nest underneath it at the top end, and some folks are allergic to/afraid of bees. Two is that it can be vertigo inducing, and three is the real danger of falling through. There is, however, a solution ... if you so desire, you can take the pathway to the right (upward) of the bridge. Right behind the rocks where people sit when they are resting at that spot.
Once at the top, don't forget to see the back side of the mountain ... on a clear day you can easily see Molokai.
For those of us who are a bit longer of tooth, the trip down can often take nearly as long as the trip up. For me, up is usually about 45 minutes, and down about 30.
The more in-shape (round IS a shape!), the entire round trip can be done in 30-35 minutes.
It can be HOT out there, so bring plenty of water. Enjoy.

I waited to climb Diamond Head before posting to use it as comparison. This trail is pretty strenuous not recommended for anyone with knee issues or stair issues. Going up is a lot of work, coming down is a lot of concentration.

Took about 35 minutes each way plus I spent about 20 minutes at the top

Great to see the whales and other islands to the SE, Waikiki to the NW.

What a great hike!

I was told by one of the locals that was climbing the same day we were there, that there were 1048 railroad ties each way. Sort of daunting when you think about it, but if you go in the morning, the shade is right for a cool hike up and AMAZING views of KoKo Crater and Hanauma Bay.

The gal who told us about the number of railroad ties, also told us of a path to divert off of a bridge that is about half-way up. For those afraid of heights, it is a good option.

What was great about this hike, is that it wasn't crowded. Those who were there were locals, or locals bringing their guests for a hike. Everyone was so friendly and willing to share the stories and history of KoKo Head and how the rails were used during WW2.

It is not a hike for someone who is not in shape. My husband has an app on his phone that can give elevation and percent grade . . . and we climbed 1200 feet at 10 to 20% grades.

Wear good climbing shoes. Don't even think of attempting in flip-flops.
