We had lunch at a lakeshore restaurant and then had a guided motor boat tour of the Isletas. It's nothing exciting, but it is a fun excursion. Having an English speaking guide is a big plus, as he had all kinds of stories about the owners of the various islands, and was good at finding interesting plants and animals. If you have the time, this is worth doing, but it is hardly the highlight of a visit to Nicaragua.
