You need to buy a ticket before you get in line,because you can't pay at the door. Climb up to the top, the tiny steep stairs that go between the dome and inner dome are amazing. You can actually imagine the builders climbing the same steps. Before you go out to enjoy the view enjoy the amazing frescos of heaven and hell inside the dome (this was one of my favorite parts, very graphic). The view when you reach the top is amazing! A great view from all directions. I recommend you get in line early, because it can be very warm, and if your claustrophobic probably you should just go in the basilica.
