Stick to meat... The Fiorentina is very good and the side dishes excellent. Also a wide and very good buffet of starters.... Try the white tuscan beans and the small zuchinis. Some good desserts are also availsble..

“Great if they know you - Occasional patrons at late hours may complain”

I have been a customer of the Terza Carbonaia for around 20 years.
The pluses are their beef (Fiorentina, Filet, Robespierre, Tagliata).
Better than most their antipasti.

In particular their hand cut near the bone Parma Ham -- prosciutto crudo all'osso tagliato a mano -- is outstanding. They also offer small beef meatballs in tomato sauce of which you may well ask for a second helping.

I tend to shy away from some of the more touristy things like fried melanzane (eggplant in the U.S. Aubergine in the U.K) and the like which some reviewers have complained about.

Terza Carbonaia will offer you a bruschetta or a foccaccia or both along side their bread offering to start out with.
The restaurant can be very noisy so 8:30 PM (20.30) is not a good time to have dinner there.
I have read complaints about their staff which I can't justify.
Maybe because I am known there I have always seen their staff to be well mannered and helpful.

Restaurants in Milan are not very good at handling tourists or casual visitors.
In that respect Milan is still the small town it used to be 50 or 60 years ago even if people in Milan for the most part now speak Italian.
Up to the mid 1960's it was frequent to find that people in Milan had trouble speaking Italian because they were more accustomed to speaking Milanese dialect.
This has virtually disappeared today. Some say because of TV or simply because of there being so many more people that have moved to Milan from other places.

Restaurants for the most part (and Terza Carbonaia is such a place) offer menus in numerous languages and waiters that can cope with more than one language.
Their wines are OK and for the most part Tuscan.
Some are a tad expensive but in general fairly priced.
I don't believe in house wine so choose it at your own risk.
For a family meal, a dinner for two or a business meal Terza Carbonaia is excellent.
Book ahead and if they tell you that they are full go someplace else.
There will always be another occasion.
Terza Carbonaia most certainly does deserve a visit.
