I absolutely loved this place, and am surprised by the lackluster reviews.
After surveying the menu options at practically every restaurant in Santa Margherita Ligure in a craving-induced quest for penne arrabiata, I stumbled on Cesarina happenstance as the manager was unlocking the front door (I thought he was the owner, but he later told us the owner is the Chef). I scanned their menu then declined his exuberant invitation to dine there, stating I was desperately seeking arrabiata. "No problem, we will make for you. Whatever you wish, we will make." he insisted, an offer I couldn't refuse. It was not quite the dinner hour yet, the restaurant did not open for an hour, but he invited us to have aperetif inside until the kitchen opened, we declined but noted the gracious offer and returned for dinner a couple hours later.
The meal was excellent, beginning to end. The appetizers, wine, and service were all impeccable, but the real highlight was the homemade impromptu penne arrabiata, which was absolutely spectacular. Easily one of the best pasta dishes I have ever indulged in - it was nothing short of amazing.
I highly recommend this place. And if they don't have what you're looking for on the menu, just ask. You won't be disappointed.

We've been here twice before and it's always a wonderful experience. The key to it is simply to trust the owner and let him tell you what he is going to prpeare. If you try and to do otherwise you mightbe disappointed by the experience. Instead, let him tell you what he is going to bring you. This goes for everything including the wine. Our meal was 11o Euros, pricey? yes. But wonderful. If you're a bit squeamish about some seafood, this is the place to get over it. Squid, Shrimp, scampi, mussels, clams etc. which can be dicey in other resturants are all done in a simple manner here and all are delicious. The wine was perfect, and the deserts were a bit of heaven - especially the rum cake. We can't wait to return next year.

Our hotel manager recommended this restaurant to us and we were not disappointed. It's a little out of the way from the tourist area, but hey, that usually means it's better, right?

We were a little nervous as we walked by it and absolutely no one else was there. This was around 7 PM, when we figured there would have to be people eating. As our vacation went on it became readily apparent that Italians typically eat later than 7:30, which we tried our best to acquiesce to, but we were usually just too hungry by then.

The waitress was great -- she only knew a little English, but offered to help translate the menu, supplementing with charades for the words she wasn't sure of (pointing to her head for "brain" and hopping up and down for "rabbit"). The meal itself was yummy and fun and lasted for about 2 hours.
