We are a family of four (17 & 15 yr old boys) and stayed 4 nights using the hotel's "Great Escape" package which included a good quality car. The upper floor of the Plantation Suite is two interconnecting rooms which were ideal for our family.
We emailed the hotel a number of times before the stay with questions, and always received prompt and helpful responses - and when we were there, we encountered the same level of service throughout. The Sugar Mill is very well managed, and run and staffed by people who obviously care about what they do, and intend to carry on doing it well. Overall we had a wonderfully relaxing time, and would recommend the place without hesitation.
The rental vehicles recommended by the Sugar Mill (through National) are good condition (ours was brand new) and good for getting up the steepest roads! Driving is easy, as traffic is light. Taxis are ridiculously expensive in the BVI (charge is per person, not for the journey), so a return taxi fare to anywhere can cost about the same as a days rental! The hotel will help sort out everything, whether you want taxis, rental, and anything else (eg diving)..
The hotel is relaxing, peaceful and quiet (not stuffy). That suited us, and our boys, who (after a week of sailing school) were chilled out. My view is that you could take small children there too, but Sugar Mill is not where I'd head with boisterous or lively kids looking for constant entertainment/action. That's not a criticism, it's just a personal viewpoint as a parent who plans family holidays.
We ate lunch at the beach bar twice. It was great - good food, fair prices, lovely drinks, friendly staff. The only pity is that it didn't carry on offering a drinks service all afternoon; it can get quite hot lying on a sunbed and swimming! We also enjoyed dinner twice at the restaurant. The food was very good, and we really appreciated the iced water. A bit of a struggle one night to get pudding due to a large birthday group, but overall we found it (like everything about the hotel) well run, and a class act. For breakfast we always headed out (try the North Shore Shell Museum!)

Overall a wonderful place. It's not cutting edge boutique style, just high quality, and treating its customers and itself with respect.
