The hike to Middleham Falls is well worth checking out. We booked a guided hike with Donaldson from Nature Isle Explorers (NIE Tours) out of Roseau on our cruise day, and it was truly a highlight of our trip. Be warned, the hike to Middleham Falls is a very strenuous hike over rough uneven terrain (think tree roots, rocks, wet, muddy ground, lots of up and down). You'll be trying to balance yourself the entire way. It takes about 1:30 to 1:45 total time if you are an experienced hiker. We had a small group of 3 couples in our 20s-30s, but did see some older groups making the trek. I would not recommend taking small children to Middleham Falls. If you are sure on your feet and don't mind a strenuous hike, Middleham Falls in Morne Trios National Park is wonderful.
Ti Tou Gorge is crowded, and like everyone says, probably some of the coldest water you will ever willingly get into. The short 10 minute hike to the Gorge is relatively easy, and there is a place to sit at the site. All ages (as long as you can swim) will enjoy this site. Most guides will require some sort of life preserver, and you will want it if you swim into the gorge as the current is very strong and could push under even the most experienced swimmer. There is a small shallow natural pool at the entrance of the gorge if you don't want to swim inside where the current is strong and it is deeper water. This site was used in the filming of Pirates of the Caribbean Dead Man's Chest. Very unique experience swimming in a canyon with a waterfall. If you get there, don't be afraid to get in. It's unlikely you'll come across such a unique swimming hole again.
I recommend finding a tour group (guided - best option, or a taxi familiar with the area) for both of these attractions. The roads in Dominica are very steep and wet from all of the rainfall, and difficult to drive if you are unfamiliar with the island.
