A great experience all round! This tour is run by and goes to support the local villagers (currently building a new school). It's challenging at times and, yes, you do need a reasonable level of fitness. It includes a walk through the jungle to the village (where you will leave dry clothes) and thence to the cave (gorge). The main activity is canyoning - walking through the gorge in thigh-high water - advise you wear reef shoes as the rocks are slippery and uneven and there's a bit of climbing involved (you can wear runners for the walk to the cave, which they will carry back for you). The cave is just magical - complete with bats - takes about an hour or so, after which you have lunch (which you need to bring). This is followed by a walk/float down the river (also has tricky rocky bits to negotiate), a climb up the cliff front and walk back to the village where you can get to meet some of the villagers. The guides were fantastic (very patient with the less fit amongst our group) and shared their local knowledge and customs. It's an all day tour, inclusive of two hours by bus to and from Luganville.
