Montalcino is a small town of about 5,000 people. I don't think a guide is required. Any good tourist guide book will list the places of interest. This will give you some ideas http://www.ilpalio.org/brief_montalcino.htm

There is a very well marked walk path south from Montalcino to the Abbey of Sant Antimo.

Road distance is about 12km. I would guess about two hours to walk it.

No problem. You might want to read Vanilla Beans & Brodo by Isabella Dusi which talks about her life in Montalcino and wandering the back streets.

There's a very handy leaflet available in the local tourist office (in English) that has details of some recommended walks in and around the town.
