Depends on how serious is the mobility issue. Could you explain a bit more? If the person is in wheelchair, Nikko Toshogu will be a challenge.
Nikko isn't really a great place for persons with mobility issues (it's a sanctuary in the mountains), although Toshogu can be reached by taxi (but then there are small stairs. Taiyuinbyo would be extremely difficult to do).
Kegon Falls is technically about 5,780 yen from Nikko stations.
