If you are connecting in Doha than you do not need a visa.

If you are travelling on Qatar Airways, no visa needed. All other airlines, you will need to exit airport and pay a $15 for a visa.

All transfers are made within the confines of the airport, you do not exit so no visa needed.

You travel from your arrival flight by bus to the departure/tranfer terminal and then again by bus from there to your departure flight.

As many nationalities cannot get visa on arrival in Qatar it would be impossible for them to leave the airport for a transfer!
