I think both of the above posts answer your question but I just had a little more info which I thought you may find useful.
There is only one direct flight each day from Chiang Mai-Phuket and that is flight number TG 0129. This flight leaves at 12:50 and arrives at 14:45. The other flights go via Bangkok and this stopover can greatly increase the length or your journey.
If you are planning on taking a few flights within Thailand I would recommend that you look into getting a 'Discover Thai' ticket from Thai airways. The cost is US$199 plus tax and it gives you 3 flights (you can purchase extra ones at a discounted rate too.)
We are flying Bangkok-Chiang Mai, Chiang Mai-Phuket & then Phuket-Bangkok. This would have cost us almost twice as much if we hadn't booked the discover Thai ticket.

This is a really old post to dig up Marissa, and I'm confused too now by your own question.
The going rate for the Chiang Mai to Phuket direct flight is around the US$175 mark booked online.
This direct flight with Thai Air departs Chiang Mai once a day at 12.35pm. There is no direct flight in the opposite direction (to the best of my knowledge).
Being only one (direct) flight each day, they will tend to fill up during busy periods, so if you want to avoid a stop at Bangkok (so a 2 hour flight takes 4 hours), book ahead.

You need to contact them by phone, you cannot book the Discovery Pass online. 3 different flights of your choosing for USD 209. Hope that helps
