Are you just sharing your itinerary or is there a question in here somewhere?

With 42 days I think you should plan more than 2 days in Siem Reap... the Angkor Park is huge and there are many temples ... check out www.canbypublications.com and www.travelfish.org for more info

airasia has a direct flight from chiang mai to phuket- saves a lot of time not changing in bangkok. it leaves CM at 1310 so gives you a leisurely morning in CM and you still get to walk on the beach before sunset!
