I was looking at car rentals for Costa Rica and the prices are as low as $5 a day at the San José airport. This can’t be right. What is the real price including insurance for a car rental? Looking fir small 4 wheel drive

Read the Car Rental Guide from the Top Qns about CR on the right side of the screen- https://www.tripadvisor.com/ShowTopic-g291982-i813-k12179446-Car_Rentals_The_Ultimate_Guide-Costa_Rica.html#96887141 . It is a long, informative thread, followed by pages of other posts related to this topic. You might want to research it all. Car rentals, combined with mandatory insurance, are not inexpensive and certainly not US$5.
Oh dear the $5 folk are still at it - yes read the car rental bible as suggested - you can figure around $500 a week in high season and $400 a week in low season but depends a lot on what insurance you choose or what you may have from a credit card like Amex premium for example,

I'm an Australian and did lots of research including reading the whole very helpful guide on here that someone took time to put together. I can assure you there is nothing close to $5 a day. For me to get full insurance, which to me is crazy NOT to get, its costing just over $700 US for 8 days. Its a cray price but I would just not risk it. I chose Vamos based on information here. Good luck!
Yes with full insurance at least $700/week in high season - figure much stuff here is about 20% off.

Just returned from 12 days in Costa Rica, SJO to Arenal area to Manuel Antonio and back to SJO. Rented from Vamos, extremely satisfied with their price, clarity, service, and support. They are clear about pricing and insurance requirements. We took advantage of their $5/day cell phone rental that included WAZE (GREAT!), unlimited incoming international calls (our son had an emergency and reached us that way), and hot spot, which we used in remote areas. They answered every question and were very responsive. We'll use them again.
We have rented a few times . Usually a 2WD SUV. For 10 days to two weeks on different trips over the years. With mandatory SLI we have paid average 50 to 65 a day based on season . Adobe, Budget, Avis and National.
