Europe to Grand Cayman Island

Am due to travel there for business later this year. While the trip to GCM is an easy call (BA via NAS), the return is more challenging as on the day I wish to travel, there is no BA flight. This seems to involve a stop in the horrendous MIA... Two questions:

- Is immigration (and Esta) still required on a international transit in the USA?

- Is there a creative way to avoid MIA considering the only OneWorld route linking GCM to the US seem to be the AA GCM-MIA?

1. Yes, but if you route through NAS and then MIA (not sure if this is possible), you will pre-clear US immigration and customs at NAS. Still need ESTA though.

2. Probably if you choose a different carrier.
