The hotel is easy to find as it is right on top of the station its not worth the pickup charge save your money for ramen and trinkets. You can walk out of the North exit of Tokyo Station and the entrance to the hotel is right through the north atrium. If you find yourself turned around and outside the street entrance to the hotel is on the north side.
You'll definitely want to take the NARITA express its much cheaper than a direct taxi and attendants at the airport can help you find your way to it. Everyone is very helpful and polite in Japan.
Google maps works in Japan is really accurate. Just keep in mind if you don't have good roaming unlimited plan you may want to pick up a local sim card.
Disneyland you can get to from the Tokyo Subway station which has multiple entrances also right under the hotel. The signage is all in english too.
Don't take a private mini car. Its really expensive and your missing one of the best parts of exploring Japan which is seeing it from the eyes of a local on the amazing transportation system. The trains/subways/shinkasen are fast, economical and efficient.

You can arrange for a meeting service when arriving by train and if there is any cost it will be minimal and worthwhile. Getting to Narita is expensive by cab or car service, the hotel arranged it for our group of 4 in a nice van and it cost about $240. Taking the train is certainly cheaper and supposed to be easy, we couldn't do it because one member of our group has difficulty walking.
