We went to Carlsbad Caverns in New Mexico last year and these caves are much more impressive. Just do it. We went to Paradise Cave which is very impressive and still active and very much alive. We also went in the river cave (Phong Nha) which is very impressive. We had a tour that picked us up at hotel in Dong Hoi. Recommended.

Our guides Tam & Cui took very good care of us throughout the high adrenaline adventure ! You have to be in good shape , have no phobias of dark caves , small spaces, or climbing over/ under/ around slippery rocks! All of the equipment was provided like: helmets, Petzl head lamps, life jackets, inflatable 'Ducky' kayaks & paddles, protective long sleeve 'water shirts', and even water proof sandals. We had to kayak 1.5 km against current at first . We arrived at a private beach deep within the cave , faraway from all tourists. We were completely alone for most of the tour with the exception of 3 other adventurous souls and our 2 guides. We also climbed/ walked , crawled, swam, and even rappelled our way to our lunch stop 1.5 kms further inside. The guides took us across an underground river after eating to this spot where they have us clay to cover our faces , arms and chest(facial). Then they had us lay down in the cool soft sand and covered us up to our necks! They left us alone for 10 minutes and turned off their lamps. Total peace and darkness ! Wow! Highly recommended only for the adventurous , fearless and good physical shape.
