Hello Kimberly and thank you for your questions. I am so sorry for any confusion! I wonder if there was just a miscommunication on the term "simple"? We do have clients who ask for a simple or standard massage and this would be the 1 hour Swedish. I am guessing that you are actually referring to the "petite" massage (30 min.)? If so, I truly apologize for the miscommunication. I can see why that was confusing! In regards to forms, it is just our policy for all massage and facial treatments, but you are always welcome to simply give your signature to allow us permission to perform the service. If you have no health concerns, etc. then do not worry about filling out the entire form.
