I love this place! One of my favorites. True Taino culture and history here. Self-guided tour. Take as long as you want. See the cave and then enjoy some time capturing the breath taking waves of the Atlantic Ocean.

The simple answer is that this excursion is a good trip for kids of all ages IF there is "proper" adult supervision - holding hands at all times for young kids. There are a lot of holes (literally) that one wrong step...

Very likely the ladder will be available. The locals that operate the area and charge the $5 entrance fee are there nearly everyday. I do not think you have anything to worry about.
