You can get on or off the bus at any one of the stops, i spent an hour at Westminster, the choice is yours

It’s included but you can get the same tour without it (which I did) that is cheaper. I did the London Eye separately and got a better deal as you can get it with a package to Madame Tussaud’s or another attraction.
