Found the hotel Natucci via excellent reviews on Tripadvisor. I booked it for 5 nights for a group of 23 Australians exploring Tuscany. Everyone found the experience memorable.
The hotel is close to all facilities, at the front desk Vincenzo is very helpful and attends to every request, the rooms are simple but clean and have all facilities. In the kitchen Vincenzo's wife Maria prepares the best of Tuscan dishes.
We had some celebrations during our stay and a suitable cake, champagne etc. were organised with no fuss.
Best of all, the price was extremely competitive. It is unbelievable that the hotel only qualifies for a 1 star rating.

For a hotel with 1 star this hotel offers a great value for money ratio.
Nice breakfast, clean and tidy room, and friendly and service minded staff, who will go out of their way to help out.

Only drawback is that the rooms are sparsely furnished.

As I understood this hotel qualifies for 3 stars if it wasn't for some local tourist organisation politics.

So if you are looking to stay in Montecatini Terme to enjoy the spas, or looking for a well placed base for a trip around Toscana, this is a hotel for you.

Stay at Albergo Natucci when going to Montecatini Terme! This hotel is clean, spacious and beautiful. Vincenzo, Maria, Anita, Guiseppe and Mrs. Ilda are very gracious and wonderful people that truly care about their customers. The food is homemade and delicious. Absolutely the most wonderful experience in Italy!

First of all, if you're looking for a great little hang-out in Tuscany, Montecatini Terme is the place! Right on the train line between Pisa and Florence, the town is famous for its mineral waters, spa treatments, and health centers. Plus it's just a great place to relax! There's a lovely park, a funicular up to the picturesque town of Montecatini Alto on the hilltop, and it's within easy train rides to Pisa, Lucca, and Florence.
Having said that, if you go, by all means check out (and check into) the Albergo Natucci. This is a lovely family-owned business where you will be treated with all the Italian warmth and friendliness you could ever need. Vincenzo, María, and their daughter Anita run a neat, pleasant Albergo and go out of their way to make you feel right at home.
The rooms are clean, standard European-sized (not huge, but not cramped either) and comfortable. They serve an ample continental style breakfast, with a few "American" add-ons like cereal, so you can start the day all fueled up and ready to go. The rates are very reasonable and affordable, plus there's all that Italian love coming your way!
A suggestion if you arrive by train: get off at the Montecatini Centrale station, not Montecatini Terme, and you'll have a shorter walk to the hotel (just a few blocks--the town isn't all that big).
If you're going to Montecatini Terme, I highly recommend "The Big Hotel Natucci".*
