Must-do? I would say some research on your own first if you want some meaningful feedback. Dave gave you excellent links to start.
There aren't really must do's, but Kaiyukan Aquarium is nice. Shopping around Namba and Dotombori is recommended. Korean barbecue in Tsuruhashi.
Ramen Museum is popular among some visitors. If you like Whiskey, you might go to the Suntory Factory in Oyamazaki between Osaka and Kyoto.
Nara's Horyuji is less visited by foreigners, but it's probably the most valuable historical heritage that Japan has.

Re Dotombori, is there a time of day to go to get in just before the crowds pile in - late Sep/early Oct? I don't mind eating really early.
Dotonbori is never filled with wall-to-wall people, certainly nothing like sardine-packed. In fact, it's fun to have some people around. Going in the late afternoon to the evening is the best time.
Dotombori and Shinsaibashi-suji arcade could be busy, but not packed crowded to the point that's annoying.

Hi, just thinking again about Osaka ...
I'm planning on leaving Kyoto to Osaka around noon on a Tuesday.
Early morning on the Thursday I fly out of KIX. So, was thinking of an airport htl on the Wed night.
So maybe I should just stay 2 nights at the Nikko Htl at KIX and go into Osaka early and spend 1 long day there on the Wednesday.
I looked up some methods of travelling in and out of Osaka and came up w' the Kansai Airport limo which does a 1 day return for Y2,000 http://www.kate.co.jp/pc/index_e.html. However, I don't know any of the limo stops in relation to wandering around Osaka and ending the day at Dotombori before heading back to the Nikkio.
Is the limo the best deal/idea as it seems I won't get value out of a 7 day JR pass just going NRT - Kyoto - Osaka - KIX.

How early is your flight out of KIX? If you are going to change hotels and stay at Osaka just for 1 night, you might as well stay put at Kyoto, then move to the KIX airport hotel for your final night. Kyoto and Osaka are roughly 30-45 minutes from each other depending on your transport method and the areas you are coming from/going to. In that case, just go to Osaka on Wednesday.
There are many ways to go from Osaka to KIX, including JR, Nankai and limobus. If you are leaving from Namba area, Nankai Airport Express (890 Yen) or Rapit (1,390 Yen, nicer train, reserved seats, luggage rack, 10 minutes quicker) are the best options. If you are leaving from Osaka station (Umeda area), then choose JR or Limobus.
The flt out of KIX is at 10am. I just think I'll sleep better if I'm at the a/port on the last night and can just walk from the htl straight into the a/port.
I'm tending to just go from KIX to Osaka for 1 long day and ret to KIX late in the evening before the flt.
I just read about a 2 day Osaka pass. What I need I think is a 1day Osaka pass. The other poster said it's a "Osaka Unlimited 2 day pass and can be used to get into the eg the Umeda Sky bldg. (2700 yen for attractions, subways and buses)"
Is there a similar pass for 1 day that would suit me from KIX - Osaka - KIX for 1 day?

If you are just visiting Osaka for one day, I recommend against going to Umeda Sky Building. It's quite a walk from Umeda/Osaka station area, and the view is average at best. Save your time and leg strength for something else.
