I apparently need to pay $25 to bring a "carry-on," but I'm allowed a free "personal item."

Does anyone know how strict they are with this? Can I bring the world's largest purse, for example?

Very. If it fits under the seat it is a personal item and free. If it has to go in the overhead it is a carry-on and costs $25 in advance. If you try to sneak a carry on onboard as a personal item and they catch you it is $100.

Do they at least have a tester thing at the airport so I can prove I'll be able to stuff it under before I get to the gate and risk an attendant saying it won't?

Yes they do, though note that the photo below shows the old fees. They have them at the gate as well.
